For patients struggling with unexplained urinary problems, a new diagnostic facility at the Central Government-run Safdarjung Hospital, Delhi, could offer a more precise answer to a question that often complicates treatment — is the problem in the bladder, the nervous system, or both?
The Centre for Advanced Bladder Diagnostics, inaugurated at VMMC and Safdarjung Hospital on Thursday, has integrated Video Urodynamics (VUDS) with Near Infrared Spectroscopy (NIRS)-based brain mapping, an approach being attempted for the first time in India.
The significance of the facility lies not merely in adding another diagnostic machine but in its ability to examine the bladder and brain response together. This could be particularly useful for patients whose urinary symptoms are linked to neurological conditions and in whom the underlying cause may not be obvious through routine tests, said a senior neurologist at the hospital.
Urinary problems such as frequent urination, urgency, difficulty in passing urine, leakage or incomplete emptying can have several causes. While some may arise from abnormalities in the bladder or urinary tract, others can be associated with conditions affecting the nervous system, including neurological disorders. Distinguishing between these causes is important because treatment can differ substantially, he added.
The new facility uses VUDS to examine how the bladder stores and releases urine while simultaneously using NIRS to assess changes in blood oxygenation in the brain during bladder filling and emptying.
This combined assessment may help doctors determine whether a patient's symptoms are primarily bladder-related, neurological or a combination of both.
For patients, this could translate into a more targeted diagnosis and, potentially, more appropriate treatment rather than repeated investigations or treatment based only on symptoms.
The centre is expected to cater to around 5,000 patients annually, expanding access to specialised urological diagnostics within a government hospital. The facility is particularly significant for patients who may otherwise have limited access to sophisticated investigations because of their cost or availability in the private sector, said the neurologist.
Union Health Minister JP Nadda inaugurated the centre along with newly installed PET-CT and SPECT-CT facilities in the hospital's Department of Nuclear Medicine.
The PET-CT facility is expected to strengthen diagnosis and treatment planning for cancer patients, including disease staging and assessment of response to treatment. It will also support specialised clinical care involving medical and radiation oncology, haematology, urology, nephrology and paediatric oncology.
For a cancer patient, PET-CT can provide doctors with information beyond what conventional imaging may show by helping identify areas of abnormal metabolic activity. This can assist in determining how extensively a disease has spread and in planning treatment.
The facility will also support follow-up after treatment, helping clinicians assess whether a disease is responding or whether further evaluation may be required.
The newly installed SPECT-CT will expand diagnostic capabilities in cardiology, renal diseases, endocrinology, orthopaedics and gastrointestinal conditions. By combining functional imaging with anatomical information, it can help doctors obtain a more comprehensive picture of certain diseases.
The advanced nuclear medicine services are expected to benefit around 600 to 700 patients annually and will be offered at an affordable cost.
The additions are particularly relevant in a public hospital setting where demand for specialised diagnostic services is high. Access to PET-CT and SPECT-CT within the government system could reduce the financial burden for patients who require such investigations as part of complex treatment.
The new bladder diagnostic centre also has an important role beyond patient care. The facility will support postgraduate education and training, allowing medical students and specialists to gain experience with advanced diagnostic techniques.
Officials said the combined facilities would strengthen multidisciplinary care at VMMC and Safdarjung Hospital, allowing different specialities to work with more detailed diagnostic information when managing complex patients.
The launch comes at a time when healthcare is increasingly moving towards precision diagnosis, where identifying the exact cause of a problem is as important as treating its symptoms.
For someone experiencing urinary difficulties, for instance, simply treating the symptom may not be enough if the underlying problem is neurological. Similarly, in cancer care, knowing not just where a tumour is but how extensively it has spread can influence the choice and intensity of treatment.
The integration of bladder testing with brain mapping therefore represents an attempt to look beyond the urinary tract alone. By observing what happens in the brain as the bladder fills and empties, doctors may gain additional clues about the communication between the nervous system and the bladder.
The facility, however, is a specialised diagnostic service and is not intended to replace conventional evaluation. Doctors will determine which patients require such advanced testing based on their symptoms and clinical condition.
During his visit, Nadda also reviewed the Heart Command Centre and Sports Injury Centre and took stock of ongoing projects at Safdarjung Hospital. He emphasised the need for regular review of projects, periodic inspection and realistic timelines so that new healthcare facilities become functional without unnecessary delays.
For patients, the larger importance of the expansion lies in bringing sophisticated diagnostics into a major public hospital at an affordable cost.
As specialised diagnostics become increasingly important in managing complex diseases, facilities such as these could help bridge the gap between advanced medical technology and affordable public healthcare, said an official from the Ministry.
