World's First Synthetic CBD Oral Therapy For Anxiety Gets CDSCO Nod, Offering New Treatment Option

As anxiety disorders emerge as one of India's fastest-growing yet most under-treated mental health conditions, patients may soon have access to a new treatment option. The country’s top drug regulator Central Drugs Standard Control Organisation (CDSCO) has approved what is being described as the world's first fully synthetic cannabidiol (CBD) oral solution for the management of mild to moderate anxiety disorders, potentially expanding treatment options for millions of patients living with one of India's most common mental health conditions.

The approval has been granted to Zenara Pharma, in collaboration with Leiutis Pharmaceuticals LLP, for the manufacture of a 150 mg/mL synthetic cannabidiol oral solution, which is to be prescribed only by psychiatrists and used in conjunction with cognitive behavioural therapy (CBT). The approval follows successful Phase III clinical trials conducted in India, with a Phase IV post-marketing study planned to generate long-term safety and effectiveness data.

The development comes at a time when anxiety disorders continue to pose a major public health challenge. According to the National Mental Health Survey (2015-16) conducted by the National Institute of Mental Health and Neurosciences (NIMHANS), anxiety disorders affect 2.57 per cent of Indian adults at any given time, while the lifetime prevalence is 3.5 per cent. Based on India's current adult population, this translates to more than 2.5 crore people living with anxiety disorders, many of whom remain undiagnosed or untreated.

The treatment gap is equally concerning. National surveys have estimated that between 28 per cent and 83 per cent of people with mental illnesses do not receive appropriate treatment because of inadequate awareness, a shortage of specialists, social stigma and limited access to mental healthcare services.

Against this backdrop, psychiatrists say the availability of a new therapeutic option could improve treatment adherence, especially for patients who require additional interventions beyond counselling and existing medications.

Unlike cannabis-derived medicinal products, the newly approved formulation contains fully synthetic cannabidiol and does not use any cannabis plant material during manufacturing. According to the developers, cannabidiol is a non-psychoactive cannabinoid and does not possess the abuse or dependence potential associated with psychoactive cannabis compounds.

The approval is also significant from an innovation perspective. Leiutis Pharmaceuticals said the medicine represents the first regulatory approval globally for a fully synthetic cannabidiol oral solution for anxiety disorders. The product has been researched, developed, patented and clinically evaluated entirely in India.

The formulation combines a synthetic cannabidiol active pharmaceutical ingredient (API), developed by Biophore India Pharmaceuticals, with a proprietary nanodispersible drug-delivery platform manufactured at Zenara Pharma's regulatory-compliant facilities.

Chandrasekhar, Chief Executive Officer and Managing Partner of Leiutis Pharmaceuticals, said the approval reflects nearly a decade of research into cannabinoid science, including the development of a proprietary synthetic cannabinoid manufacturing process, innovative drug-delivery technology and extensive pre-clinical and clinical evaluation.

"This approval marks an important milestone for our innovation pipeline and demonstrates India's capability to develop globally relevant therapies from discovery through regulatory approval," he said.

Dr. Jagadeesh Babu Rangisetty, Chief Executive Officer of Biophore India Pharmaceuticals, described the approval as evidence of India's growing pharmaceutical innovation ecosystem.

"The journey from concept to approval required scientific perseverance and close collaboration at every stage. It reinforces India's capability to deliver globally relevant pharmaceutical innovation," he said.

Experts caution, however, that the medicine should not be viewed as a standalone treatment. The CDSCO approval specifically states that the drug is to be used alongside cognitive behavioural therapy, reflecting the continued importance of psychological interventions in managing anxiety disorders.

The medicine is also not intended for over-the-counter sale and can be prescribed only by psychiatrists, ensuring specialist evaluation before initiation of treatment.

Mental health specialists point out that anxiety disorders encompass a range of conditions—including generalised anxiety disorder, panic disorder and social anxiety disorder—that can significantly impair quality of life if left untreated. Persistent anxiety is associated with reduced productivity, disturbed sleep, cardiovascular complications and an increased risk of depression and substance-use disorders.

The approval also signals India's growing role in developing novel therapies for neuropsychiatric disorders. Besides anxiety, Leiutis Pharmaceuticals said it is expanding its research pipeline into pain management and oncology using proprietary drug-delivery technologies and cannabinoid-based therapeutics.

While the regulatory approval represents an important scientific milestone, experts say the larger challenge remains improving early diagnosis, reducing stigma surrounding mental illness and ensuring equitable access to specialist mental healthcare.
