You Can't Scream In Space
The blame is sticking rather stubbornly to NASA’s lapels and citing "technical errors" as the cause for the fatal disintegration of Columbia on February 1 won’t be enough for the space research agency to rescue its reputation. A report by the Columbia Accident Investigation Board (CAIB), which was made public last week, confirmed that while the shuttle broke up because a part of the foam insulation had come unstuck during the lift-off, funds-starved NASA’s inefficient internal culture is the actual culprit. The report adds it was the casual attitude of the NASA hierarchy, which refused to take the snag seriously, even though it had the precedence of the 1986 Challenger crash to go by, that failed to save India-born astronaut Kalpana Chawla and her six companions.
